Several people and projects use CentOS as a base within which to build additional functionality. We note them alphabetically.
These are NOT CentOS. We merely collate a listing here to provide a reference target. Some may well be a forked Vserver variant, or a downstream fork under the tests of the Vserver page.

AsteriskNOW
AsteriskNOW is an open source Software Appliance based on CentOS. Don't ask help on CentOS channels but seek help from them... quoting AsteriskNOW 1.5.0 released: "As always, please use [http://bugs.digium.com/ ] and http://forums.digium.com/ for issues and feed."
BlueOnyx
BlueOnyx is a sub- or successor fork of BlueQuartz, it seems
BlueQuartz
BlueQuartz was a community successor to the commercial Cobalt Qube and Raq distribution and products bought by Sun Microsystems, and then abandoned in fairly short order. It appears to be abandoned since 2007 as to updates, as of early 2009. Part of its updates were pulled from CentOS or upstream SRPMs
CactiEZ
CactiEZ is based on CentOS-4. See http://cactiusers.org/bugs/view.php?id=119 for more details on their update policy.
CentServer
CentServer takes CentOS-5/rpmforge packages and repack/distribute (maintain?) their own version and repository. I (TruHuynh) could not find the corresponding src.rpm
ClarkConnect
ClarkConnect is based on CentOS but use their own repositories (see for instance the centos rpms in [http://redhot.clarkconnect.com/community/5.0/System/RPMS/ ])
Elastix
Elastix is based on CentOS-5 but requires the CentOS-5.1 kernel! forum post as of 2008/10/20.
FAN
FAN (Fully Automated Nagios) goals is to provide a CD based on CentOS...
OS Office
OS Office OSPanel theme for BlueOnyx Hosting provider that supplies an OS based on CentOS 4.8 but with an el5 kernel. See discussions on the Forum.
PIAF
PBX in a Flash is based on CentOS... but has it's own custom installer and post-install scripts ans it's own way to update. (from http://knol.google.com/k/ward-mundy/pbx-in-a-flash/3uqc77rg9tgar/2# "If you update your CentOS configuration, you will need to reinstall it by running update-scripts, then update-fixes, and then install-netconfig."-> ask **them** if you break something!
R-fx add-on packages
R-fx adds packages which are reported in IRC to not play well with CentOS IPTables and packet filtering model and configuration tools.
SME Server
SME Server is an outgrowth from the former Mitel 'e-Smith' in-house Linux distribution, which was set free back in the 'Red Hat Linux' days.
Snaplogic
Snaplogic (and its parent) seems to use a later Python version and make pervasive changes.
trixbox (formerly Asterisk@Home)

trixswitch
The trixswitch ISO install package is a CentOS-based CD-ROM image that creates a boot CD that will automatically install a CentOS variant and FreeSWITCH.
